To MANUALLY locate the SP&P guide in the main Broward County School website http://www.browardschools.com and click the RESOURCE TAB it will be on your right hand side, it says: Special Programs and Procedures PDF. Another way
is to look for the FDLRS RESOURCE. It's page 5 within the resource tab...make sure you look for
the numbers at the bottom of the page and click on page 5...then look for Special Programs & Procedures...but if you want

Medwaiver INTAKE form. Print and fill out to get signed up on
the MEDWAIVER WAITLIST thru the APD.
*When your child is 3 years old, you can sign them up for the Medwaiver Program. Contact your local
APD office for more info.
The 5 qualifying eligibilities to be on the Medwaiver Program are:
1. Mental Retardation
2. Cerebral Palsy
3. Autism
4. Prader-Wili
5. Spina Bifida
