When you decide to take a DS Advocate to your next IEP meeting:

 

 

OUR POLICIES & PROCEDURES FOR SERVICE

 

We will be there for you. We do expect you to take an active roll at your childs upcoming IEP meetings. In order for us to help you help your child receive all the supports and services, and have a well written ACADEMICALLY SOUND IEP written, there is HOMEWORK to be done way before your next scheduled IEP meeting.

 

In order for us to do our jobs to the best of our abilities as your advocate, we do need you to supply us with certain things:

  1. What is it you hope for us to accomplish for you.
  2. What are the 5 main concerns you have for your childs education and educational placement.
  3. PLP ~ aka Present Level of Performance. Parents will do their own informal assessment on their child and email us that information to take to upcoming IEP meetings.
  4. Word Lists ~ Parents use flashcards, books, toys to come up with their childs current word list. How many pictures can the child identify, how many words can your child speech? In English and other languages. How many words does your child know in Sign Language. All this information gives a better picture of just how well and how much the child knows and can communicate.
  5. Evaluations and Assessments done on the child all compiled in a Word Document. This organizes any and all evaluations that have been done on your child.
  6. PPF ~ aka Parent Participation Form FAXED to:                              (954) 495-4615 BEFORE Parent signs and commits to a date and time of upcoming IEP meeting. (*IMPORTANT - if this is not done properly there is no guarantee that a DS Advocate can attend your upcoming IEP meeting)   
  7. Release of Information Form filled out and given to the school. (2 copies made, one for parent and one for DS Advocate)   
  8. Retainer funds received before DS Advocate will attend any IEP meetings. 
  9. Contact information of ESE Specialist and address of school where meetings will be held. Detailed driving directions provided to DS Advocate or email a mapquest to school address.  
  10. Phone numbers and email address.

*If several of the above LIST OF items are not provided in a timely manner, DS Advocate reserves the rights to discontinue services at anytime. Please adhere to the few requirements our program requests in order to serve you better.

Phone consultation gets us started after that we set up a date and time to come out and meet your beautiful child, we can't advocate effectively if we have never met your child. That is how we get started helping you learn new advocacy skills.
 
With our advocacy services, over time you will see how much you have learned and your confidence level will improve at 'speaking up and speaking out' at those many many IEP meetings you will be attending for years to come.
